Emergency Notice
CornerstoneHealthMD is not for emergencies. If you are experiencing an urgent or emergent medical condition, dial 9‑1‑1 immediately or go to the nearest emergency department.

1) Agreement to Terms

These Terms & Conditions of Use (the “Terms”) govern your access to and use of CornerstoneHealthMD’s websites, mobile applications, messaging, scheduling, payment pages, and related services (collectively, the “Platform”). By creating an account, booking or attending a visit (including telemedicine/video, phone, chat, or asynchronous services), or otherwise using the Platform, you agree to these Terms, our Privacy Policy, and our Notice of Privacy Practices (NPP) (collectively, the “Agreements”). If you do not agree, do not use the Platform.

2) Who We Are

“CornerstoneHealthMD,” “we,” “our,” or “us” refers to Cornerstone Health MD, P.C., and/or its affiliated professional entities and administrative management company. Licensed clinicians (“Providers”) deliver clinical care; CornerstoneHealthMD itself does not practice medicine. Clinical judgment and the provider–patient relationship rest solely with the Provider.

3) Services We Provide

CornerstoneHealthMD offers telemedicine and select in‑person services in:
– Addiction Medicine (e.g., medication for opioid use disorder, alcohol use disorder),
– Medical Weight Management, and
– Virtual Urgent Care.

Services may include evaluation, counseling, prescribing when appropriate, care coordination, and ordering labs/imaging. The scope of services may vary by state, your clinical needs, your insurance/self‑pay status, and applicable laws.

4) Eligibility; Age; Residency

You must be 18 years or older (or the age of majority in your state) to use the Platform, unless accompanied by and with consent of a parent or legal guardian where permitted. You must be physically located in a U.S. state where our Providers are licensed at the time of your visit and you must confirm your location during each telemedicine encounter. Services are not available outside the United States.

5) Telehealth Consent & Risks

By using telehealth services, you acknowledge and consent to:
– the nature and limitations of telemedicine (e.g., possible incomplete information due to technology limits; delays; equipment failures; rare security risks despite safeguards);
– Providers may decide that in‑person evaluation, testing, or follow‑up is required;
– you may be asked to provide photos, vitals, home devices data, or complete labs; and
– you may withdraw consent to telehealth at any time (this may affect the ability to continue care).

6) Controlled Substances, PDMP & Program Policies (Addiction Medicine)

CornerstoneHealthMD complies with all applicable federal and state laws, medical board rules, and DEA requirements related to prescribing controlled substances and telemedicine. You understand and agree that:
– Providers will check your state Prescription Drug Monitoring Program (PDMP) as required. – Identity verification is required; you may be asked for a government ID at each visit. – Urine drug screens (UDS), breathalyzers, pill counts, and/or treatment agreements may be required and periodically repeated. – Certain medications (e.g., benzodiazepines, stimulants, or other Schedule II–IV drugs) may be declined or limited via telemedicine based on clinical judgment and applicable law. An in‑person exam may be required before or during treatment. – Diversion, falsification of information, threatening behavior, or non‑adherence to program policies may result in discharge and/or termination of services.
– Refills are never guaranteed and are based on Provider judgment, safety, and compliance.

7) Weight‑Management Program Disclosures

Prescription weight‑loss medications (including GLP‑1s and other agents) are used only when clinically appropriate and consistent with labeling and guidelines. You may be required to complete baseline labs, monitoring, and follow‑up. Medication availability and pricing may change; we do not guarantee coverage or supply.

8) Virtual Urgent Care Limitations

Virtual urgent care addresses non‑emergent conditions. If symptoms suggest an emergency (e.g., chest pain, shortness of breath, stroke symptoms, severe injury, suicidal thoughts), you will be directed to call 9‑1‑1 or go to the nearest emergency department.

9) Not Insurance; No Guarantee of Results

CornerstoneHealthMD is not an insurance plan and does not replace your primary care provider or emergency services. Medical outcomes cannot be guaranteed.

10) Your Account & Security

You are responsible for maintaining the confidentiality of your login credentials and for all activities under your account. Notify us immediately of any unauthorized use. You agree to provide true, accurate, current, and complete information and to update it as needed. We may suspend or terminate access for suspected misuse or security concerns.

You consent to electronic delivery of required disclosures and to electronic signatures for consents and forms.

No Mobile information will be shared with third parties/affiliates for marketing/promotional purposes. If a Provider prescribes a medication or orders tests, you may use a pharmacy or lab of your choice where legally permitted. CornerstoneHealthMD is not a pharmacy and does not have ownership interests in pharmacies or labs. We do not guarantee fulfillment, pricing, or turnaround times by third parties.

18) Pricing; Payment; Self pay -No Insurance

We may offer self‑pay options where available. You agree to pay all fees, copayments, coinsurance, and deductibles due. Prices and eligibility may change. Some services require pre‑payment. If your payment method fails or is disputed, we may suspend access until resolved and may charge reasonable fees permitted by law. Itemized statements may be available upon request.

19) Refunds; Cancellations; No‑Shows

Unless stated otherwise in a posted policy, prepaid visits are refundable prior to the clinician beginning the encounter. Appointments canceled within 2 hours of the scheduled time or missed without notice may incur a no‑show/late‑cancel fee. Medication, lab, and device purchases are typically non‑refundable once ordered/dispensed.

The Platform and any content are provided “as is” and “as available.” To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, title, non‑infringement, accuracy, and uninterrupted or error‑free service.

24) Limitation of Liability

To the fullest extent permitted by law, CornerstoneHealthMD and its affiliates shall not be liable for any indirect, incidental, consequential, special, exemplary, or punitive damages, or lost profits/data, arising from or related to your use of the Platform or third‑party services. Our total liability for any claim arising from these Terms or the Platform shall not exceed $500.

25) Indemnification

You agree to defend, indemnify, and hold harmless CornerstoneHealthMD, its affiliates, Providers, officers, directors, employees, and contractors from any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to your use of the Platform, your violation of these Terms, or your violation of any rights of a third party.

26) Dispute Resolution; Arbitration; Governing Law

Before filing a claim, you agree to contact us to attempt to resolve the issue informally. Any dispute arising out of or relating to these Terms or the Platform that cannot be resolved informally shall be resolved by binding arbitration on an individual basis under the rules of the American Arbitration Association (AAA). Class actions are waived. The Federal Arbitration Act governs interpretation and enforcement of this provision. If arbitration is deemed unenforceable, the dispute shall be brought exclusively in the state or federal courts located in Gwinnett County, Georgia, and you consent to their jurisdiction and venue. Jury trial is waived to the extent permitted by law.

Sections that by their nature should survive termination (e.g., IP, payment obligations, disclaimers, limitations, indemnity, dispute resolution) shall survive.

30) Entire Agreement; Severability; Assignment; Waiver

These Terms, together with the Privacy Policy and NPP, constitute the entire agreement between you and CornerstoneHealthMD regarding the Platform. If any provision is found unenforceable, it will be modified to the minimum extent necessary, and the remainder will remain in effect. You may not assign your rights; we may assign ours as permitted by law. Failure to enforce a provision is not a waiver.
